What is Hospice

Hospice care is a special kind of care focusing on the quality of life for people and their caregivers who are experiencing an advanced, life-limiting illness. Hospice care provides compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible.

The hospice philosophy accepts death as the final stage of life: it affirms life, but does not try to hasten or postpone death. Hospice care treats the person and symptoms of the disease, rather than treating the disease itself. A team of professionals work together to manage symptoms so that a person’s last days may be spent with dignity and quality, surrounded by their loved ones. Hospice care is also family-centered – it includes the patient and the family in making decisions.

Registered Nurse

In collaboration with the Medical Director and entire care team the RN directs the care each patient and family receives; RN provides ongoing support, direct care, education, medication assistance, symptom management and more as it relates to end of life.

Social Work

Provides patients and families ongoing emotional and psycho-social support; provides education and counseling related to grief and end of life; helps connect patients and families to resources in community; works in collaboration with entire care team.

Aide

Provides companionship and activities of daily living (ADL) support for patients up to 7 days a week depending on patient’s needs; works in collaboration with RNCL.

Spiritual Care Coordinator

Provides patients and families ongoing spiritual and emotional support; provides education and counseling related to spirituality, grief, and end of life; helps connect patients and families with spiritual leaders and resources in the community; works in collaboration with entire care team.

Physician/Medical Director

Ensures all patients receive excellent individualized care; responsible for the overall palliation and management of the patient's terminal illness; works closely with RN and supports entire care team to oversee care plan, medications, symptom management, level of care and more.

Volunteer

Provides additional support for patient's, families, and staff.  Volunteers available for companionship, vigil support, administrative support and more.

Medications and Durable Medical Equipment

Medications related to the patient's hospice diagnosis or intended to control or alleviate pain and associated symptoms along with equipment necessary to provide a comfortable, safe and caring environment are included in our services.

Costs for Care

The cost for our services and all items such as equipment, medications, etc is completely covered under a patient's Medicare Hospice benefit.  If a patient does not have Medicare but is covered under a private insurance plan payment for services will be negotiated with the insurance company.  Lumicare Hospice patients and their families will not receive a bill for any covered services.
